Notes on Making Appointments:
- Please fill out the form below and a member of our staff will contact you to schedule an appointment.
- In an emergency situation, contact your physician immediately or call 9-1-1.
- Silicon Valley Spine Institute treats spine and neck related conditions.
- Please be sure to bring all medical records, x-rays and any other related information with you to your appointment. The doctors will need to see the actual x-ray, CT or MRI films a the time of your visit, so it may be necessary for you to contact the establishment where these films were taken prior to your visit and making arrangements to pick them up.
- You are also strongly encouraged to bring your insurance card and a government-issued ID.
- If you have not heard from our office within 24 hours, please call our office at 408-376-3300 to schedule an appointment.
